Authorized healthcare providers, hospitals, case managers, and community agencies can submit referrals through our secure HIPAA-compliant referral form. Our team carefully reviews each referral to determine placement suitability.
Yes. We use a HIPAA-compliant referral process and maintain appropriate safeguards to help protect sensitive health and personal information in accordance with applicable privacy and security requirements.
Yes. We assist residents with budgeting, meal preparation, housekeeping, personal care, transportation planning, appointment management, and other daily living skills to promote greater independence.
Transportation may be available for medical and other approved appointments. We also assist residents with arranging Medicaid or other community transportation services when needed.
